Online Software Disclosure Form
This 'Online Software Disclosure Form' is only for a software for which already a licensee is known. e.g an ETH Zurich Spin-off, a company or a research institution.

Important information:
- The person completing this form must be an employee of ETH Zurich, but not necessarily a developer of the software. The developers will not receive an e-mail with the completed form. After the form has been submitted only the responder to this form will receive an e-mail with a '.pdf' of the completed form. Such '.pdf' must be signed by all developers and by the responsible professor(s) and the signed copy sent to . It is responsibility of the responder to share the completed '.pdf' form with all the developers and the responsible professors(s).
- To access the online software disclosure form, you need an ETH Zurich account, and prior to access the online page you are required to enter your 'username' and 'password'.
- Only a single person at time can access and complete the online disclosure form for each disclosure form. Sharing the form between different persons/accounts is currently not possible.
- You can move back and forth between the different sections and questions until you submit the online disclosure form. And, whenever you enter the link below, you will always land on the page where you closed/exited the 'tab' or the 'browser'.
- Important Note: If the “cache” or the “data” of your browser is deleted/emptied when you close/exit the browser (e.g. Firefox), the information entered in the form will be totally lost. Therefore, we recommend that you deactivate the “Delete cookies and site data when Firefox is closed” checkbox in the browser settings so that nothing is lost when the browser is closed.
- You can open a second disclosure form only if you submit the first one that you have already opened.
- To delete the entire information of a disclosure form, please go the top right end of the form and select the three dots '...' next to the image of the book.
- After the disclosure form was submitted, you will receive a '.pdf' the of the disclosure form. Please sign such document and collect the signatures of all developers and the responsible person(s). The '.pdf' document can be signed electronically.

Required information in the online form
In the form, i.a., we will require to complete the following information:
- A. Information about the licensee (e.g. name and address of the licensee; possibly contact person; preferred license type).
- B. Information about each developer of the Software (name, e-mail, contribution percentage, etc.).
- C. Information about the Software
- Title;
- Description;
- Whether the software contains data or databases;
- Technical requirements;
- Development effort (hours and number of lines of code).
- D. Embedded or accessed third party code (a list of the third party code must be uploaded as a supplementary document).
- E. Export Control
- F. Financial Support and Rights of Third Parties.
- G. Dissemination of Software.

A copy (fac-simile) of the software disclosure form generated by the system, a copy of the entire questionnaire and the information you are required to enter into the form, and templates for the list of third-party codes are available on the form's access page (see link below).
